Our client is looking for an Assessment Care Coordinator for servicing clients in the West End of GTA (Oakville-Mississauga Border)
What we offer
- Contract date: ASAP until March 31, 2023
- Location: Mississauga – Oakville, Ontario L6H 0H2
- Pay: $ 23.20/hr. plus 4% vacation pay
- Hours of Work: 8 AM to 8 PM and they run 7 days a week. They work a 40-hour week and Flexibility required to work a rotation of shifts including holidays and weekends
What you do:
- Processing referrals, including conducting assessments and determining eligibility, to ensure appropriate placement in the SDL and CRS Services respectively.
- Developing trusting relationships and requires resourcefulness and professionalism to assist parties to access the necessary community resources to meet their needs.
- Conducting or reviewing (interRAI-CHA, Caregiver Assessment and other assessments) either in-home or in hospital
- Prioritizing referrals and managing the waitlist as necessary
- Ensuring timely and effective linkages to service providers or alternate resources
- Tracking and reporting activity levels for both programs
Who you are:
- Post-secondary education in relevant health or social service field
- Valid Ontario Driver’s license and a reliable insured vehicle
- Needs to undergo a vulnerable sector check
- Must be fully vaccinated
- Must pass a crim check
- Experience with conducting formalized client assessments (preferably interRAI CHA or RAI-HC)
- Ability to build positive rapport with various referral sources and to professionally represent all SDL /CRS service providers
- Working knowledge of MS Word, Excel, PowerPoint and other software tools. Experience with GoldCare client record system a strong asset.
- Excellent communication skills (verbal, written, listening, comprehension)
- Excellent customer Service Skills
