JOB SUMMARY
The Garrison Crossing Home Care Coordinator is responsible for the delivery of resident care for older adults utilizing the nursing processes of assessment, planning, implementation, and evaluation according to regulations and best practices of Community Care and Independent Living (as per legislative regulatory bodies). The Home Care Coordinator must maintain competencies-based skills of knowledge, skill, attitude, and judgment to provide safe care. He/she also ensures the care reflects and complies with Elim’s overall philosophy of care. The Home Care Coordinator is also a part of the EHCS Leadership team.
QUALIFICATIONS
Þ Full scope LPN required.
Þ Registered with the British Columbia College of Nurses and Midwives (BCCNM).
Þ Minimum of 5 years experience in Community Care, Supported Living or Assisted Living preferred.
Þ Good working knowledge of the regulations established by Community Care, the Assisted Living Registrar and other regulatory bodies related to senior living.
Þ Proficient at computer programs and advancing electronic health records
Þ Current First Aid and CPR certificate.

Þ Desire to be a daily difference maker in the lives of older adults
Þ Demonstrated clinical competencies and leadership skills.
Þ A genuine empathy for older persons, an ability to relate meaningfully with them, and a desire to effectively care for them including their mental, physical, social, emotional, and spiritual needs.
Þ Evidence of good physical and emotional health and the capability to handle the demands of the job.
Þ Good working knowledge of provincial legislation and programs to promote compliance with regulatory reporting requirements
Þ Demonstrated ability to work harmoniously to develop and foster a team approach with members of a multidisciplinary team.
Þ Ability to respond effectively in emergencies within BCCNM’s scope of practice.
Þ Effective leadership skills to anticipate needs and delegate accordingly.
Þ Ability and desire to support the organization’s philosophy, values, goals, policies, and procedures.
Þ Ability to work effectively in an ever-changing resident focused environment.
Þ Comfortable making care and service plan recommendations to current/potential home care clients
