General Summary

The HIM Clerk will be responsible for receiving and transmitting all ROI requests at the Health Center of Southeast Texas. The HIM Clerk will work under immediate supervision and will maintain patient documents in order, respond to requests for medical records, and perform other applicable clerical duties as needed. The right candidate will be a highly motivated person with an eye for details and the ability to get the job done with minimal errors. The HIM will be part of the Health Information Management team and will be required to handle HIT/IT issues as well with guidance from HIT Assistant.

· Knowledge of HIPAA and other federal and state regulations relating to patient files, medical records, billing records and patient registration.

· Receives, prepares, and transmits medical record requests for all HCSET locations.

· Respond to external and internal calls related to release of information and record requests.

· Organizes and evaluates medical records for completeness and accuracy before requesting approval for release.

· Pull and route records to appropriate personnel or department.

· Knowledge of chart control, access, and storage in accordance with established policies and regulations.

· Knowledge of medical records regulations and release of health information

• Assist with departmental audits and investigations

· Log medical record requests into ROI Spreadsheet using Excel

• Determine records to be released by reviewing requestor information in accordance with HIPAA guidelines and obtaining pertinent patient data from various sources, including electronic, off-site, or physical records that match patient request. Scan medical record requests and fax confirmations into EMR Database.
• Assist with other applicable PHI Document management systems
• If necessary, respond and process requests from physician offices on a priority basis and fax information to the physician office.
• Comply with site facility policies and regulations.
• Escalate issues to manager that delay timely release of medical records
• Perform other position related duties as assigned.
