Under the supervision of the Telehealth Supervising Coordinator, serves as an integral member of the telehealth team performing daily equipment install/repair/de-install visits, providing patient education, screening daily patient health information transmissions and to provide technical support to patients and staff. This position must be able to demonstrate the knowledge and skills necessary to provide service appropriate to the age of the patients served in their area. This position requires technical savvy – must be comfortable / proficient with servicing medical equipment and machines in patient homes*This position requires strenuous physical activity – frequent lifting of equipment in and out of vehicles, in and out of buildings, including occasionally up and down stairsThis position consists of approximately 50% in the office, at the hospital and 50% in the field, driving to and from patient homes TELEHEALTH PROGRAM ASSISTANCE Schedules own appointments with patients in caseload, meets with patients in their homes Provides patient and caregiver education on telehealth equipment, including use and care of equipment as well as importance of using equipment for disease management. Enters and maintains patient information in the telehealth patient portal. Reviews patient information and health trends in the patient portal and contacts patients when readings are out of desired range or data is missing. Communicates patient information and any concerns to the RN Calls for emergency assistance (such as 911) in the event of medical emergency in the patient’s home. TELEHEALTH EQUIPMENT MANAGEMENT Performs installation, de-installation, and troubleshooting and testing of telehealth equipment in patient homes. Sanitizes returned telehealth equipment, per applicable infection control protocol, prior to returning to inventory. Performs inventory tracking and maintenance. Returns equipment to manufacturer for replacement when appropriate Assists Telehealth Supervising Coordinator with other program duties as assigned -High School Diploma or GED equivalent required-BLS for the Healthcare Provider completed by 60 days after hire required -Ability to work well independently as well as part of a team required -Strong customer service skills required, including (but not limited to) patience, compassion and being observant and pro-active -Valid Michigan Driver’s License with no restrictions and no more than one traffic violation in the last 3 years required -Must be able to provide own transportation, company car not provided-Six months experience as a Nursing Assistant / Tech in a hospital, home care agency or healthcare related setting, or enrollment in a healthcare related program preferred-Certified Nursing Assistant Certification (CNA) preferred
