JOB TITLE: Program Coordinator
DEPARTMENT:Medicaid Services
PROGRAM:Nursing Home Transition and Diversion (NHTD) Waiver
SUPERVISOR:Director – Medicaid Services
STATUS: Full-time, Non-exempt
SUMMARY
The Program Coordinator supports the Director in triaging referrals, assigning team members and assists the Director with Quality Assurance activities. This position communicates with internal and external referral sources to gather additional information to assist in prioritizing assignments. The Program Coordinator also carries a caseload of no more than 12 NHTD clients and is responsible for the development, implementation, and monitoring of those client service plans. Clients and caregivers are linked to services and opportunities that are available within their community. Guidance identifying care options and navigating challenging systems is provided to clients and caregivers.
DUTIES AND RESPONSIBILITIES
1.0 Review and process all NHTD program referrals
1.1 Review all referrals
1.2 Triage and assign all referrals to the team
1.3 Communicate with referral sources, as needed
1.4 Update client system database
2.0 Provide Care Management
2.1 Accept case assignments
2.2 Develop, implement, and maintain individual service plans
2.3 Coordinate services for individuals with chronic health conditions that qualify for long-term skilled nursing care
2.4 Refer and link individuals and their care partners to community-based services
2.5 Document all services delivered in the designated database
2.6 Monitor data relating to Medicaid status and service plans
2.7 Successfully complete required hours of annual professional development trainings in accordance with oversight
agency.
3.0 Assist with Quality Assurance activities
3.1 Participate in program chart reviews
3.2 Communicate with Director any quality assurance concerns
4.0 Participate as a member of the inter-agency team
4.1 Participate in all Lifespan staff meetings and trainings
4.2 Participate in all program meetings and trainings
4.3 Participate in after-hours telephonic on-call rotation
4.4 Accept and seek guidance and supervision
5.0 Timely completion of all required documentation and data entry into appropriate database.
6.0 Maintains up to date professional knowledge of the geriatric field, with special focus on community services, entitlement benefits, clinical skills and competencies through reading, workshops, and conferences.
7.0 Is aware of and actively supports the Lifespan’s initiative of being collaborative and community focused, innovative and culturally competent.
8.0 Other duties as assigned by supervisor.
Requirements
QUALIFICATIONS
EDUCATION:Master of Social Work, Bachelor of Social Work or equivalent degree in the fields of health and human services.
EXPERIENCE:At least two years of experience coordinating community-based services for older adults. Working knowledge of
Medicaid Home and Community Based Services.
PHYSICAL AND MENTAL REQUIREMENTS
· Must be able to meet with clients in their homes.
· A portion of the work is performed at a desk or at a personal computer station on Lifespan premises.
· Verbal, written and telephone communication skills are required.
· Strong written and computer skills needed.
· Interface with all levels of personnel.
· Ability to maintain composure under strict deadlines and difficult situations.
· Ability to work in multi-cultural situations.
· Access to reliable transportation.
· Valid NYS Driver’s license
