Job Description
The Care Coordinator functions as an integral part of the Care Connection team as one of the primary contacts for his/her assigned patients. The primary focus is on engaging and retaining new patients into primary care within the organization. This includes helping his/her assigned patients be prepared for their visits within the ambulatory setting, greeting and guiding patients to the appropriate area of care, helping patients to understand billing information, and working collaboratively with clinical/non-clinical personnel to identify barriers and find solutions to create a seamless experience for patients. The Care Coordinator supports the Care Connection Nurse Navigator, Physicians, and other Care Connection team members by coordinating the process of care to our patients through every aspect of care.
Minimum Requiremen t s
High School diploma or equivalent required.
At least one year of experience in clerical or administrative capacity of health care or customer service setting.
Must possess computer keyboarding skills of at least 30 wpm and basic knowledge of Microsoft computer programs.
Ex c ellent interpersonal and communication skills
Must be able to read and speak the English language
Considerable ability to provide superior customer service.
Preferred:
Proficient in Microsoft applications
Community service experience
Multi-language skills
