Community Partners Hiring for Supported Living Provider at Dover, New Hampshire Full Time

Seeking individual or couple in the Strafford Country area, to act as Supportive Living Provider(s) to a 33 year old man, with Angelman Syndrome and a Severe Intellectual Disability . There are currently two options for the living arrangement. The first option is the Supportive Living Provider would take over the lease of the two-bedroom apartment in downtown Dover, where the individual has lived with his prior Supportive Living Provider. The second option is that the individual would move into the Supportive Living Provider’s home. This would require a first floor bedroom, as the individual has some difficultly with stairs. Additional details regarding the living space would need to be reviewed. This individual has a full-time day program which will take him out of the home M-F from 10:00am – 5:00pm, with additional respite hours some evenings. Individual also spends 2 nights a week (Saturday night and Sunday night) at his family’s home, to allow for scheduled respite on a weekly basis. Supports to include: grocery shopping and all meal prep, full assistance with all personal care and ADL’s, administration of all medications, and 24/7 supervision. Ideally no children under 10 or small pets. Experience working with individuals with physical and/or developmental disabilities preferred. Individual is part of the Family Directed Services program, which will be discussed in further detail at the time of interview. A Home Providership is a contractual agreement, with a generous stipend, that may be an alternative or supplement to a more traditional employment opportunity. Our non-profit Agency provides program oversight, and offers educational/clinical training and support to ensure success.
