Job Description
JOB SUMMARY
Provide intensive case management services for individuals with severe and chronic mental illness. Services are provided in the home, or at locations convenient for the client. Provides proactive outreach to the severely mentally ill, facilitation and support in assisting them to meet their basic needs, and linkage to formal mental health services. Services will be provided up to 24 hours daily.
QUALIFICATIONS
EDUCATION
A diploma or degree in human service-related discipline.
Demonstrated thorough comprehensive knowledge of severe mental illness and treatment, and its impact on consumer/survivors and their families, which includes the effects of poverty and marginalization.
KNOWLEDGE/EXPERIENCE
Minimum 2-3 years experience working with the severely mentally ill population, preferably in a community setting.
Personal and/or familial exposure to mental illness would be advantageous.
SKILLS
Skills in counseling, addictions, housing, community outreach, concurrent disorders and general aspects of physical health are an asset.
Must demonstrate excellent communication skills, both verbal and written, and knowledge of local community resources.
Computer skills an asset.
SPECIAL CONDITIONS
Must demonstrate an acceptable comfort level in working with individuals with severe mental illness, and be capable of maintaining objectivity in order to perform consistently with the philosophy and objectives of the Intensive Community Support Program.
Must possess and maintain a valid driver’s license to the class of vehicle operated
Work flexible hours as required in response to community based health hazard.
