Career Description Summary: The Scheduler will schedule patient visits, assist the DON and ADON in monitoring schedules and productivity. This position will also reschedule and assign case management as needed.

Essential Functions & Responsibilities:

Works with DON and ADON to receive scheduling needs information and with field staff to receive second calls when DON and ADON are not available

Ensures that the needs of the patient are the priority and that schedules are not changed to meet staff needs.

Maintains tracking per policy.

Notifies DON or ADON when any type of visit cannot be performed by the RN Case Manager.

Ensures that patients are not seen by multiple individuals from the same discipline if possible.

Runs daily and weekly reports as requested and notifies the DON or ADON of any issues that arise.

Lives the PLUS Standards in the care of patients, daily tasks, and interactions with others.

Maintains regular attendance and punctuality which are critical in order to complete the day-to-day tasks of this position.
