SummaryThe Patient Services Coordinator supports the Patient Services Manager (PSM) and other staff with receiving and processing patient grievances and provider concerns. The Patient Services Coordinator also oversees other patient related activities as needed, such as the patient mentor program. Responsibilities for the Patient Services Coordinator also include supporting emergency preparedness activities, developing patient and provider educational resources, and assisting in coordination of Patient Subject Matter Expert activities.Essential Competencies, Duties and ResponsibilitiesSupports the PSM with receipt, documentation, and resolution of patient and family grievances regarding the quality of dialysis care received at dialysis facilities.Assists the PSM with facility notifications and correspondence and completing staffing updates and provides technical assistance related to requests or concerns.Recruits, monitors, and supports the Networkis Patient and Family Subject Matter Experts (SMEs) and Beneficiary and Family Engagement Learning and Action Network (LAN) activities.Provides primary oversight, support, and guidance for the Networkis Peer Mentoring Program.Oversees, with support from the Executive Director, Emergency Preparedness activities, including collaboration with Emergency Operations staff, KCER and CMS before, during and after an emergency or disaster. Supports Patient and Family Engagement (PFE) related projects as needed, such as providing PSM assistance with PFE technical assistance activities.Develops educational offerings, including WebEx presentations and workshops to provide opportunities for facility staff members to gain knowledge regarding conditions for coverage, requested topics, and special programs provided by various professionals in the field as appropriate. Such WebEx offerings will be clear, concise and relevant to the education level of the participants.Develops and promotes Network resources to stakeholders.Ensures that timelines are met as set forth by CMS.Contributes to required administrative reports, e.g., monthly report to CMS and annual reports.Ability to effectively communicate with ESRD beneficiaries, physicians, administrators, and other individuals on grievances and access to care issues, as appropriate.Ability to adhere to strict CMS and HSAG confidentiality guidelines.Project deadlines may necessitate occasional extended work hours.Additional TasksIs accountable for accurate and timely completion of all assigned functions and tasks.Must possess the ability to work weekends and holidays as required by the contract.Must possess the ability to be flexible in adjusting work hours to workload to assure timely completion of assignments.Is accountable for appropriate communication and problem-solving behavior with health care providers, ESRD beneficiaries, CMS, and other professional staff.Must possess the ability to communicate, present, and teach, through written and verbal means, information in a pleasantly clear, concise and accurate manner.Must possess the ability to use individual discretion and professional judgment to initiate actions.Must possess the ability to work independently or as part of a team.Attends staff meetings and in-service presentations conducted at HSAG.Education and/or ExperienceAt minimum a bacheloris degree in social work.Three (3) years of social services experience.Quality improvement experience is desirable.Renal experience is desirable.Other QualificationsComputer literacy skills using Microsoft applications, es
